The Certificate of Origin is a kind of certification document that proves the origin of goods, that is, the place where the goods are produced or manufactured. Effect
The certificate of origin (CERTIFICATE OF ORIGIN) is a kind of certification document issued by the exporter at the request of the importer and issued by the notary agency, government or exporter to prove the origin or place of manufacture of the goods. The Certificate of Origin is a valid evidence for the transfer of goods by the trader, settlement of payment, claims settlement, customs clearance of the importing country, and collection of tariffs. It is also a proof that the exporting country enjoys quota treatment and the importing country implements different trade policies for different exporting countries.
According to China's relevant regulations, companies must apply for a certificate of origin to a visa agency no later than three days before the goods are declared for customs clearance. In strict accordance with the requirements of a visa agency, the following materials must be truthfully, completely, and accurately filled in:

(1) Certificate of Origin (CERTIFICATE NO.)
This column must not be left blank, otherwise the certificate is invalid.
(2) Exporter (EXPORTER)
Enter the full address, name, and country (region) name of the export company. If you need to fill in the name of the importer through other countries or regions, you can fill in English VIA after the exporter, and then fill in the name, address and country of the re-exporter.
(3) Consignee
Fill in the final consignee name, address, and country/region name. It is usually the buyer or letter of credit specified on the letter of credit. If the letter of credit stipulates that all the consignee’s column is left blank, in this case, this column shall be marked with “TO WHOM IT MAY CONCERN” or “TO ORDER”, but this column must not be left blank. If you need to fill in the name of the re-exporter, you can add the English VIA after the consignee and then write the name, address, and country of the re-exporter.
(4) MEANS OF TRANSPORT AND ROUTE
Fill in the port of shipment and port of destination, and the mode of transportation. If transshipment, transit sites should also be indicated.
For example: By sea, transit from Shanghai Port to Hamburg Port via Hong Kong, should be filled in: FROM SHANGHAI TO HAMBURG BY VESSEL VIA HONGKONG.
(5) Destination Country (Region) (COUNTRY/REGION OF DESTINATION)
Fill in the destination country (region). Generally it should be consistent with the final consignee or final destination port (territory) country, and can not fill in the intermediaries country name.
(6) FOR CERTIFYING AUTHORITY USE ONLY
It is used by a visa agency after issuing a certificate, issuing a certificate, or adding other statements. The certificate application unit should leave this column blank. Under normal circumstances, this column is not filled.
(7) MARKS AND NUMBERS
Fill in the gimmick. The complete pattern, word mark and package number should be filled out according to the letters and letters listed on the letter of credit, contract and invoice. You cannot simply fill out "AS PER INVOICE NUMBER" or "AS PEER B/L NUMBER". If there is no bread in the goods, the words "NO MARK" should be filled in. This column should not be left blank. If there are many items in this column, please fill in the blanks in columns 7, 8 and 9. If it is still not enough, you can fill it in the attached sheet.
(8) NUMBER AND KIND OF PACKAGES;DESCRIPTION OF GOODS
Fill in the description of the product and the quantity of the package. The name of the product name must be filled in with a specific name, and no general statement, such as clothing, food (GARMENT, FOOD), etc., shall be used. The quantity and type of packaging should be filled out according to specific units, which should be strictly consistent with the letter of credit and other documents. The number of packages should be stated in English after the Arabic numerals. If the goods are in bulk, add the word "IN BULK" after the name of the goods. Sometimes the letter of credit requires that the contract number, letter of credit number, etc., be added to all documents, and can be added in this column. The last line of this column should be marked with the symbol indicating that the content should be added.
(9) Product Code (H.S. CODE)
This field requires the HS code, which should be consistent with the declaration form. If the same certificate contains several products, the corresponding tax number should be filled in. This column must not be left blank. Otherwise, the certificate is invalid.
(10) QUANTITY
This column requires the quantity of the exported goods and the unit of measure of the goods. If there is only gross weight, you need to fill in "G.W."
(11) Invoice number and date (NUMBER AND DATE OF INVOICE)
Fill in the commercial invoice number and date. This column must not be left blank, otherwise the certificate is invalid.

(12) DECLARATION BY THE EXPORTER
Fill in the name of the exporter, the place and date of application and the signature of the person who has registered at the visa agency and have a seal in both English and Chinese.
(13) Signed and stamped by a visa agency (CERTIFICATION)
Fill in the visa location and date. Visa agency visas are verified in this column (original) after verification and a visa stamp is issued.
